This form acknowledges that I have signed up for an “SV Ayurveda Educational Training/Workshop/Course for Self-Improvement” education module with SVAyurveda.com, Adishakti Inc, Chandi Co and its consultants/facilitators. SVAyurveda which originates from Vaidya Rama Kant Mishra’s family tradition is an ancient educational practice that originated in India. In the United States, SVAyurveda is part of complementary and alternative medicine (CAM). I acknowledge that I am in good physical health and I will not use SVAyurveda as a replacement for proven conventional care or postpone seeing a doctor about a medical problem. I will tell my health care providers about any complementary and alternative practices I use. I will give them a full picture of what I do to manage my health to help ensure coordinated safe care.

Use of this site is provided by Adishakti RM Inc/SVAyurveda/SVA is subject to the following terms and conditions:

Your use constitutes acceptance of these terms and conditions as at the date of your first use of the site.

1. Adishakti RM Inc/SVAyurveda/SVA reserves the rights to change these terms and conditions at any time by posting changes online. Your continued use of this site after changes are posted constitutes your acceptance of this agreement as modified.

2. There are no refunds on donations/course registration fees after signing up for a course/material and signing the Terms And Conditions Form.

3. You agree to use this site only for lawful purposes, and in a manner which does not infringe the rights, or restrict, or inhibit the use and enjoyment of the site by any third party.

4. This site and the information, names, images, pictures, logos regarding or relating to Adishakti RM Inc/SVAyurveda/SVA are provided “as is” without any representation or endorsement made and without warranty of any kind whether express or implied. In no event will Adishakti RM Inc/SVAyurveda/SVA be liable for any damages including, without limitation, indirect or consequential damages, or any damages whatsoever arising from the use or in connection with such use or loss of use of the site, whether in contract or in negligence.

5. Adishakti RM Inc/SVAyurveda/SVA does not warrant that the functions contained in the material contained in this site will be uninterrupted or error free, that defects will be corrected, or that this site or the server that makes it available are free of viruses or bugs or represents the full functionality, accuracy and reliability of the materials.

6. Copyright restrictions: Commercial use or publication of all or any item displayed is strictly prohibited without prior authorization from Adishakti RM Inc/SVAyurveda/SVA. Nothing contained herein shall be construed as conferring any license by Adishakti RM Inc SVAyurveda/SVA to use any item displayed.

I acknowledge that this course and/or educational module content has not been evaluated by the FDA (Food and Drug Admninistration) and it does not prescribe or treat any disease, illness, or injury. The educational material may include recommendations and/or guidelines for diet and lifestyle changes, specific herbal formulations and body work technique treatments. These guidelines are considered intrinsic to the SV Ayurveda body of knowledge and are not meant to be used to treat, cure, or diagnose any identified illness or disease. No medical treatment is offered or given. I understand that the SVAyurveda Educator/facilitators reserve the right to discontinue a registrant’s privileges at their sole discretion. In such cases, the tuition fees that have been paid in full or in part will be refunded. Other expenses that may have been incurred in this context, such as transportation, accommodation, products, etc. are NOT subject to reimbursement. The course/consult fees are non-refundable unless extenuating circumstances are cited (inability to attend the event and prior at least 24 hour notice given). I understand that I am not authorized nor will I endeavor to appropriate materials and teach SV Ayurveda protocols without prior full certification and licensing. I will not use it in my practice without certification and I will not use SV Ayurveda knowledge in any way without proper identification and source citation. I HAVE READ and I understand the foregoing memorandum, which outlines the scope of the education module I have signed up for. I specifically understand that the education module is NOT offered by a licensed medical doctor(s), or physician, and that it will not cover the discussion of medical treatments of any kind. Neither the information, products recommended (when any) nor body work techniques/massage are intended to treat, mitigate, cure, or prevent any disease. All information provided will be for the sole purpose of imparting education on SV Ayurveda and the SV Ayurvedic protocol of products and techniques recommended. I acknowledge receipt of this memorandum. I hereby release, indemnify, save and hold harmless SV Ayurveda, Adishakti Inc, Chandi Co. and its “SV Ayurvedic health and well-being” educators/facilitators, their companies, owners, directors, officers, employees, agents, assignees, subsidiaries, and licensees, (hereinafter referred to as “Adishakti”) from and against any costs, fees, expenses, liabilities, or claims arising from any activity, treatment or treatments with Adishakti or from any conduct or activity arising from Adishakti activities involving ourselves or any conduct or activity involving any Adishakti products recommendations or body work techniques, including but not limited to illness, injury, death, theft, or other liability, regardless of any act or omission from Adishakti or his invitees or licensees. As a material part of the considerations to participate in such activities with Adishakti, I hereby assume all risks of injury to persons or damage to property arising from any cause, and I hereby waive all claims against Adishakti or her invitees or licensees. I am assuming the risk of all known and unknown claims I may have. In this regard, I waive the benefits of any state or federal or international statute that may allow protection against unknown or unanticipated claims, damages, liabilities, or actions, whether contractual, statutory, or tortuous in nature. Adishakti is furthermore not liable for any injury, death, theft, damage, accident, delay or inconvenience in the event that I am damaged or injured regardless of the cause. By signing this memorandum, I intend it to be a complete and unconditional RELEASE of all liability to the greatest extent allowed by law. If I, or anyone else on my behalf, makes claim against Adishakti Inc. or the consultants, I, or my estate, will INDEMNIFY, SAVE, and HOLD them HARMLESS from any litigation, expense, attorney fees, loss, liability, damage or cost which may occur as a result of such claim to the fullest extent permitted by law. I agree that if any portion of this memorandum is held to be invalid, the balance shall continue in full force and effect. This “waiver and release memorandum” shall be interpreted according to California law and all disputes arising from this form or from Adishakti’s activities shall be addressed in the jurisdiction of California, USA.

In addition, documents may be copied for personal use only on the condition that copyright and source indications are also copied, no modifications are made and the document is copied entirely. However, some documents and photos have been published on this site with the permission of the relevant copyright owners (who are not Adishakti RM Inc/SVAyurveda/SVA). All rights are reserved on these documents and permission to copy them must be requested from the copyright owners. By posting or submitting content on our site, or appearing in the video recordings of live sessions, you grant Adishakti RM Inc/SVAyurveda/SVA a non-exclusive, royalty-free, perpetual, worldwide licence to republish any material you submit to us, including your image/likeness and screen name, without limitation, in any format. You hereby authorize School/Host/SVAyurveda/Adishakti RM Inc to record and edit related materials that may carry name, likeness, image, voice and participation in and performance on film, tape or otherwise for use in promotional materials or parts thereof (the “Recordings”). You agree that content may be edited and otherwise altered at the sole discretion of the Producer and used in whole or in part for any and all broadcasting, non-broadcasting, audio/visual, and/or exhibition purposes in any manner or media, with no time constraints, globall. Producer, its successors and assignees shall own all right, title and interest, including copyright, in and to the Program, including the Recordings, to be used and disposed of without limitation as Producer/Adishakti RM Inc/SVAyurveda/SVA shall in its sole discretion determine.
